Job summary

Choice in Aging (CiA) seeks a Care Professional to lead and assist with program activities for frail elders and adults with disabilities. You will support physical and social wellbeing, work under supervision of the Activity Coordinator and Program Director, and document daily interactions and wellbeing in a respectful, confidential environment.

The role emphasizes person-centered care, teamwork, and clear communication, including ensuring safety and dignity in all activities.

Qualifications

• High school diploma or equivalent.
• Clear TB test; must pass Livescan fingerprint background check.
• Must communicate clearly in English, both verbal and written.
• Must be able to loudly project voice for hearing impaired clients.
• Must be able to work as a team member.

Responsibilities

• Lead, engage and assist with program activities to encourage client participation with respect and trust.
• Provide care respecting individuality and promoting personal worth, agency and hope.
• Build one-on-one relationships with participants based on respect.
• Understand participants' likes, interests, backgrounds and family history.
• Assist with activities of daily living including toileting, feeding, grooming, and personal hygiene.
• Provide safety supervision, emotional support, and behavior management when needed.
• Maintain a clean program area and assist with cleaning duties as assigned.
• Assist with food preparation, serving meals, and cleanup.
• Complete client documentation such as progress narratives and logs; ensure confidentiality.
