Job summary

Choice in Aging is seeking a Care Professional to lead and assist with program activities for frail elders and adults with disabilities. The role involves providing health and social services under supervision to support independence and dignity within the community.

You will document daily interactions and wellbeing, maintain confidentiality, and work as part of a multidisciplinary CiA team to meet program goals.

Qualifications

High school diploma or equivalent is required.
Clear TB test and LiveScan fingerprint background check.
Must communicate clearly in English, both verbally and in writing.
Must be able to loudly project voice and articulate clearly for hearing impaired clients.
Must be able to work as a team member.

Responsibilities

Lead, engage and assist with program activities to encourage client participation and respect.
Provide care that respects each individual's uniqueness.
Maintain personhood by promoting self-worth, agency, social confidence and hope.
Build one-on-one relationships with participants based on respect.
Assist with activities of daily living including toileting, feeding, ambulating, transferring, dressing, grooming and hygiene.
Complete client documentation and maintain confidentiality.
