EmpowerMe Wellness is seeking a Care Coordinator to support the Area Director of Operations in senior living communities. You will optimize therapists’ schedules, perform intakes, coordinate care, and host wellness trainings to ensure high-quality resident interactions.

Qualifications

• Certification in healthcare, business administration, or related field.
• Two years of relevant experience in a senior living setting preferred.
• Proven experience in administrative and operational roles, including managing schedules and maintaining records.
• Exceptional communication and interpersonal skills.

Responsibilities

• Identify health needs and build caseload with residents and families.
• Coordinate intake process and scheduling of evaluations.
• Provide care coordination services through on-site visits and wellness activities.
• Assist in coordinating therapists’ weekly schedules and onboarding of new team members.
• Review operational reports and assist in improving care coverage.
