Qualifications

• At least 2 years of health care experience including LTSS.
• Clinical licensure and/or certification as required by state.
• Bachelor's degree in a health care related field may be required.
• Valid driver's license and reliable transportation.
• Strong communication and ability to work with diverse populations.

Responsibilities

• Completes comprehensive member assessments within regulated timelines, including home visits as required.
• Develops and implements care plans, including a waiver service plan with members and care network.
• Performs ongoing monitoring of care plans and documents progress and needed changes.
• Promotes integration of services including behavioral health and LTSS.
• Assesses medical necessity and authorizes waiver services as appropriate.
• Facilitates ICT meetings for approval/denial of services and collaboration.
• Identifies barriers to care and coordinates resources to address psycho/social, financial and medical obstacles.
• 25-40% estimated local travel may be required.
