Job summary

Miami Jewish Health seeks a Home Care Coordinator to develop and implement homecare services for Florida PACE Center participants. You will coordinate Durable Medical Equipment, nursing and therapy services to ensure high-quality, patient-centered care across Dade and Broward counties.

The role requires nursing education (LPN preferred) and strong ability to work with a multidisciplinary team, ensuring safety and compliance while coordinating field visits and home assessments.

Qualifications

• Graduate of an accredited school of practical nursing preferred or higher level of education.

Responsibilities

• Assess homecare needs and develop specific plans of care.
• Coordinate homecare services including nursing, physical therapy, occupational therapy, speech therapy and personal care to meet client needs.
• Coordinate and authorize Durable Medical Equipment, consumables, and frozen meals.
• Coordinate and supervise contracted nursing care for Florida Pace Center participants in nursing homes or boarding homes.
• Assess program homecare services system and propose improvements to better meet participant needs.
• Participate in Multidisciplinary Team Meetings.
• Perform compliance monitoring of contracted home care agencies and personnel, including ALF audits for quality purposes.
• Prepares and coordinates Health Assessment Form 1823 for ALFs contracted by Florida Pace.
