Job summary

Miami Jewish Health seeks a Home Care Coordinator to develop and implement homecare services for Florida PACE Center participants, coordinating durable medical equipment, nursing and therapy services to ensure quality care.

The role involves assessing homecare needs, arranging DME and contracted nursing care, and ensuring compliance and safety across Dade and Broward counties to support a dignified aging experience.

Qualifications

• Graduate of an accredited school of practical nursing preferred or higher level of education.
• One to three years of experience working with frail elderly population and home care.
• Current LPN license by the Florida State Board of Nursing, preferred
• Current BLS Certification
• Valid Florida Driver's License and Proof of Insurance
• Must have good communication skills in English
• Knowledge of Spanish may be required

Responsibilities

• Assesses the homecare needs of participants, and identifies and develops specific plans of care
• Assist in Safety evaluations for Prospect prior to enrollment with documentation and uploading in EMR
• Coordinates the implementation of all homecare services which includes nursing, physical therapy, occupational therapy, speech therapy and personal care services to ensure that quality services is provided to meet client needs
• Coordinates and authorizes all Durable Medical Equipment, Consumables, and Frozen Meals
• Coordinates and supervises all contracted nursing care for FPC participants in the nursing home/boarding home according to program standards. Includes interaction with other facility staff
• Assesses program homecare services system, makes recommendation and offers solutions to revise or modify the system to better meet participant’s needs
• Actively participates in Multidisciplinary Team Meetings
• Performs compliance monitoring of contracted home care agencies and personnel. This includes all ALF audits for Quality purposes
• Participates in department meetings and in-services as necessary
• Prepares and Coordinates Health Assessment Form 1823 for ALF’s Contracted by Florida Pace.
