Job summary

Miami Jewish Health in Fort Lauderdale, FL, is seeking a Home Care Coordinator to develop and implement homecare services for Florida PACE Center participants. You will coordinate nursing, therapy, and Durable Medical Equipment to ensure high-quality, patient-centered care across home and facility settings.

Requirements include a practical nursing diploma or higher, 1–3 years’ experience with frail elderly populations, an active Florida LPN license (preferred), current BLS, and a valid Florida

Qualifications

• Graduate of an accredited school of practical nursing preferred or higher level of education.
• One to three years of experience working with frail elderly population and home care.
• Current LPN license by the Florida State Board of Nursing, preferred.
• Current BLS Certification.
• Valid Florida Driver’s License and Proof of Insurance.
• Excellent communication and ability to work with diverse teams.

Responsibilities

• Assesses homecare needs and develops plans of care.
• Assist in safety evaluations prior to enrollment and document in EMR.
• Coordinate homecare services including nursing, therapy and personal care.
• Coordinate and authorize Durable Medical Equipment, Consumables, and Frozen Meals.
• Coordinate and supervise contracted nursing care in facilities according to standards.
• Assess and revise homecare systems to meet participant needs.
• Participate in Multidisciplinary Team Meetings.
• Perform compliance monitoring of home care agencies and ALF audits.
• Participate in department meetings and in-services.
• Prepare Health Assessment Form 1823 for ALFs contracted by Florida Pace.
