Job summary

Responsibilities include health risk assessments, developing and coordinating care plans, monitoring services, and advocating for members and families while mentoring new team members.

Qualifications

• Master's degree & 2 years of related experience, or equivalent combinations per posting.
• Bachelor's degree & 4 years of related experience.
• Associate degree & 5 years of related experience.
• Up to 10 years of experience may substitute for education/experience requirements.

Responsibilities

• Supports health plan members and facilitates a team approach across the continuum.
• Conducts health risk assessments and develops care plans.
• Coordinates, monitors and evaluates care with the interdisciplinary team to improve outcomes.
• Acts as advocate and liaison for members and families, and mentors new staff.
