Qualifications

• Associates Degree required; 2 years related experience in lieu of degree acceptable.
• CCM certification preferred or obtainable within 3 years of hire.
• Must have valid driver license, clean driving record, able to travel locally.

Responsibilities

• Facilitates a team-based care approach with the Interdisciplinary Care Plan team to ensure quality, cost-effective care across the continuum.
• Coordinates care for clients using assessment, care planning, implementation, monitoring and evaluation.
• Provides care coordination for patients with chronic conditions and complex needs, including transitions to home care.
