Presbyterian Healthcare Services is hiring a skilled Care Coordinator II to join our care coordination team in New Mexico. The role focuses on team-based planning, cost-effective delivery, and quality outcomes across medical, behavioral, social, and long-term care services.

Responsibilities include coordinating care, supporting patients in hospital, clinic settings, and during transitions, while engaging with interdisciplinary teams to address patient needs and optimize outcomes.

Qualifications

• Associates Degree; 3 years experience can substitute for degree; Bachelor's preferred.
• 2 years of related experience.
• Valid driver license, clean driving record, and ability to travel locally.
• Experience in utilization management, quality assurance, home care, community health, long term care or occupational health.
• CCM certification preferred or obtainable within 3 years of hire.
• Proficiency in Word, Excel and Outlook.
• Experience in analyzing trends based on decision support systems.
• Cost/benefit analysis and negotiation skills.
• Knowledge of referral coordination to community and public resources.

Responsibilities

• Facilitates a team approach to ensure interventions and cost effective quality care.
• Collaborates with interdisciplinary care plan team including members, caregivers, physicians and providers.
• Coordinates care for identified populations using assessment, planning, implementation, coordination, monitoring and evaluation.
• Provides care coordination to members with chronic conditions and less complex needs.
• Performs home visits as required.
• Educates providers and staff on care coordination and health strategies.
• Maintains relationships with external stakeholders and promotes appropriate use of resources.
• Generates reports and participates in Interdisciplinary Care Team meetings.
