Job summary

Qualifications

• Bachelor's or Master's in Social Work; MSW preferred.
• Licenses: LMSW or LCSW GA required upon hire.
• Minimum 2 years healthcare experience in acute care.

Responsibilities

• Perform psychosocial assessments and develop discharge plans.
• Coordinate care across the continuum with the care team.
• Serve as consultant on psychosocial and discharge needs.
• Mentor and precept new social workers and interns.
• Document in the Electronic Health Record and track outcomes.
• Identify barriers and connect patients with community resources.
