Job summary

Wellstar Health System is hiring a Care Coordination Social Worker (CC SW) to assess transitional care needs, coordinate across the continuum, and engage with patients and families to ensure safe, timely discharges. The role includes psychosocial assessments, community resource referrals, and collaboration with RN Case Managers and the care team.

Required education includes a BA/BSW or MSW with GA licensure preference; BLS certification is expected.

Qualifications

• Excellent written and verbal communication skills.
• Strong assessment, interview, organizational and problem-solving abilities.
• Knowledge of local, state, and federal regulations related to care coordination.
• Ability to work collaboratively with physicians, care teams, and patients/families.

Responsibilities

• Perform psychosocial assessments and plan discharge in collaboration with care team.
• Coordinate with RN Case Managers, financial counselors, and UM nurses for insurance and coverage.
• Participate in interdisciplinary rounds to determine best discharge plan and transition.
• Provide resource information and referrals for community services and post-acute care.
• Document progress and ensure records are accurate and up-to-date.
• Serve as a mentor or preceptor for social work students when appropriate.
