Job summary

The Sr. Social Worker partners with physicians and RN Care Coordinators, mentors staff, and contributes to protocol development

Qualifications

• Minimum education: BSW or MSW degree (MSW preferred).
• BLS certification required upon hire.
• LMSW or LCSW licensure in GA, or eligibility.

Responsibilities

• Implements discharge planning and provides resource information for complex patients.
• Identifies and documents barriers for timely disposition.
• Partners with the care team to support discharge plans and transitions.
• Mentors new and less senior social workers; may precept students.
• Ensures timely, accurate documentation in the electronic health record.
