You will conduct wellness visits, assist with transportation, personal care, medication coordination, light housekeeping, and documentation. You’ll design personalized engagement sessions and use tablets or apps to reduce isolation and promote wellbeing.

Qualifications

• High school diploma or equivalent required.
• Experience with older adults and caregiving preferred.
• Knowledge of person-centered care and aging challenges.

Responsibilities

• Conduct wellness visits and monitor clients' well-being.
• Provide transportation to medical appointments and social outings.
• Assist with personal care, medication coordination, and housekeeping.
• Document services and client interactions accurately and timely.
• Develop engaging, personalized activities tailored to each client.
• Collaborate with Care Managers and families to support goals.
