Job summary

Meals on Wheels North Central Texas is seeking a Case Manager & Program Specialist to join the Client Services team. This year-round, full-time role is based in the Administrative office and reports to the Client Services Director.

You will assess client needs for meal service, manage ongoing case work, and provide compassionate telephone and online support to recipients, caregivers, and referral partners as they enroll.

Qualifications

• Bachelor's Degree in Social Work or equivalent.
• 2–3 years of case management experience, preferably with elderly or disabled.
• Experience working with seniors and/or people with disabilities preferred.
• Detail-oriented, organized, and self-motivated with a track record of completing assignments.
• Proficient in Microsoft Office (Word, Excel, Outlook).
• Clear written and spoken communication; professional yet approachable.

Responsibilities

• Provide case management to identify and address client needs.
• Develop support systems by coordinating services and community partnerships.
• Follow up on client concerns and maintain communications records.
• Answer general questions about Meals on Wheels for recipients and caregivers.
• Assist with client intake and annual assessments, verify eligibility for meal service.
• Screen and address referrals and update client files per status changes.
• Participate in team trainings and contribute to program development.
