Aufgaben

• Navigate patients to cancer screening, diagnostic and treatment services.
• Proactive outreach to community partners and primary care practices to identify new collaborators.
• Create revised navigation workflows and metrics that align with evidence-based guidelines.
• Train new navigation team members and provide expertise on metrics collection and reporting.
• Work closely with center leadership to identify and design new services relevant to cancer prevention and survivorship.
• Work with data manager to collate and report navigation-related metrics for internal and external reports.
