Qualifications

Responsibilities

• Conduct comprehensive assessments and medication reconciliation.
• Support members during short-term sub-acute rehabilitation episodes.
• Ensure accurate and complete documentation, including ICD-10 coding.

Skills

Active ANCC or AANP certification

Current NCCPA certification

Active, unrestricted licensure

Understanding of geriatrics

Experience in geriatric medicine

Proficiency with electronic medical records
