The Senior Clinical Pharmacist at Providence in Washington collaborates with physicians and staff to deliver coordinated, high quality patient care across the care team. The role focuses on collaborative practice, medication optimization, and population health management to improve outcomes.

Qualifications

Bachelor's Degree in pharmacy or PharmD from an accredited School of Pharmacy.
Postgraduate Year One (PGY-1) Pharmacy Residency or equivalent education/experience.
Washington Pharmacist License required at hire.
Board of Pharmacy Specialties Certification within 2 years of hire.
Board of Pharmacy Preceptor License (Facility Managed) – when eligible and where applicable.
3 years of Pharmacy practice experience, with at least 2 years of related experience in lieu of PGY-1.

Responsibilities

Patient Care – The Senior Clinical Pharmacist is responsible for providing collaborative practice services in collaboration with the healthcare team.
Population Management – The Senior Clinical Pharmacist may utilize registries for chronic diseases and reports/tools to identify target populations/patients and enhance therapeutic outcomes through disease/medication education and drug therapy optimization.
