Job summary

The Senior Clinical Pharmacist at Providence in Washington collaborates with physicians and staff to deliver coordinated, high quality patient care across the care team. The role focuses on collaborative practice, medication optimization, and population health management to improve outcomes.

Qualifications

  • Bachelor's Degree in pharmacy or PharmD from an accredited School of Pharmacy.
  • Postgraduate Year One (PGY-1) Pharmacy Residency or equivalent education/experience.
  • Washington Pharmacist License required at hire.
  • Board of Pharmacy Specialties Certification within 2 years of hire.
  • Board of Pharmacy Preceptor License (Facility Managed) – when eligible and where applicable.
  • 3 years of Pharmacy practice experience, with at least 2 years of related experience in lieu of PGY-1.

The Senior Clinical Pharmacist will collaborate with physicians and staff to provide coordinated, comprehensive, high quality care to patients. The responsibilities of the clinical pharmacist will predominantly fall into the following areas:

  • Patient Care – The Senior Clinical Pharmacist is responsible for providing collaborative practice services in collaboration with the healthcare team.
  • Population Management – The Senior Clinical Pharmacist may utilize registries for chronic diseases and reports/tools to identify target populations/patients and enhance therapeutic outcomes through disease/medication education and drug therapy optimization.
Qualifications
  • Bachelor's Degree in pharmacy from an accredited School of Pharmacy or PharmD in pharmacy from an accredited School of Pharmacy.
  • Coursework/Training and Postgraduate Year One (PGY-1) Pharmacy Residency or equivalent education/experience.
  • Upon hire: Washington Pharmacist License (Vendor Managed).
  • Within 2 years of hire: Certification from Board of Pharmacy Specialties.
  • Upon hire: Board of Pharmacy Preceptor License (Facility Managed) – when eligible and where applicable.
  • 3 years of Pharmacy practice experience with at least 2 years of related experience in lieu of Postgraduate Year One (PGY-1) Pharmacy Residency.
