Job summary
Inova Health is seeking a Clinical Pharmacist to join the Anticoagulation Clinic. This full-time role focuses on warfarin management in clinic and via remote monitoring, with a hybrid schedule and some weekend on-call responsibilities.
The team emphasizes safety, quality, and patient experience as part of a national-leading healthcare system in Northern Virginia. Sign-on bonus and relocation assistance are eligible for this position.
Qualifications
- PharmD or PharmBachelors required.
- Five years of experience as a clinical Pharmacist.
- Licensed Pharmacist in VA or eligible to obtain licensure.
Responsibilities
- Interprets, prepares, and verifies medication orders.
- Provides clinical consultation and cost-effective alternatives.
- Issues controlled substances in compliance with DEA/VA Board requirements.
- Participates in formulary review and approval.
- Monitors drug therapies for interactions and appropriateness.
- Reports adverse drug reactions promptly.
- Minimizes non-formulary procurement and supports substitutions.
- Participates in decentralized rounding with clinical teams.
- Responds to emergent bedside alerts and codes.
- Precepts learners and may perform additional duties incl. grant work and teaching.
