Job summary

Inova Health is seeking a dedicated Clinical Pharmacist to join the Anticoagulation Clinic team. This full-time role (Mon–Fri) includes a hybrid schedule with some rotating weekend on-call responsibilities and a sign-on bonus plus relocation assistance.

Qualifications

• PharmD or Bachelor degree in Pharmacy is required.
• Five (5) years of experience as a clinical Pharmacist.
• Licensed Pharmacist in the state of VA.
• BLS, ACLS for Critical Care and Emergency Medicine, and PALS within 90 days of hire.

Responsibilities

• Interpret, prepare, and verify medication orders.
• Provide clinical consultation and cost-effective alternatives.
• Handle controlled substances in compliance with DEA/VA Board requirements.
• Participate in formulary review and approval process.
• Monitor drug therapies for interactions and safety.
• Report adverse drug reactions promptly.
• Minimize non-formulary procurement and support substitutions.
• Participate in decentralized rounding with clinical teams.
• Respond to emergent bedside alerts and codes.
• Precept learners as appropriate.
• Lead grant work and contribute to pharmacy practice development.
• Lecture and facilitate learning for students and residents.

This position would join our team of clinical pharmacists in the anticoagulation service supporting Inova cardiology and primary care patients. The primary focus is on warfarin management both in clinic and via remote monitoring. This is a hybrid position Monday-Friday day shift with some rotating weekend on call responsibilities.

Clinical Pharmacist Job Responsibilities:

• Monitors drug therapies for interactions, allergies, and appropriateness.
• Reports adverse drug reactions promptly and accurately.
• Minimizes non-formulary procurement and supports substitutions.
• Participates in decentralized rounding with clinical teams.
• Responds to emergent bedside alerts and codes (adult and pediatric as designated).
• Precepts learners as appropriate.
• May perform additional duties as assigned, including, but not limited to:
  • Spearheading grant work and submission related to the practice of pharmacy.
  • Lecturing and facilitating learning opportunities for pharmacy students, PGY1 and PGY2 pharmacy residents, and practitioners.
  • Performing duties on local, system. And national committees which contribute to the development and professional advancement of pharmacy practice.
  • Serving as a leader or facilitator on a regional or national professional organization.

Minimum Qualifications:

• Education: Doctor of Pharmacy (PharmD) or Bachelor degree in Pharmacy.
• Experience: Five (5) years as a clinical Pharmacist.
• Licensed Pharmacist in the state of VA.
• Certification: Basic Life Support (BLS), Advanced Cardiovascular Life Support (ACLS) for Critical Care and Emergency Medicine, and Pediatric Advanced Life Support (PALS) within 90 days of hire.

Preferred Qualifications:

• PGY2 in ambulatory care, cardiology, or BCACP eligible
• Experience in warfarin management inpatient or outpatient, including LVAD patients and periprocedural bridging.
