Inova Health System is seeking a dedicated Sr Clinical Pharmacist to join the Ambulatory Anticoagulation Clinic in the VA, United States. Job Description

Inova Health is seeking a dedicated Clinical Pharmacist to join the Anticoagulation Clinic team. This role will be full-time Monday – Friday. |Sign-on Bonus & relocation assistance eligible.

This position would join our team of clinical pharmacists in the anticoagulation service supporting Inova cardiology and primary care patients. The primary focus is on warfarin management both in clinic and via remote monitoring. This is a hybrid position Monday-Friday day shift with some rotating weekend on call responsibilities.

Clinical Pharmacist J ob Responsibilities:

• Participates in decentralized rounding with clinical teams.
• Responds to emergent bedside alerts and codes (adult and pediatric as designated).
• Precepts learners as appropriate.
• May perform additional duties as assigned, including, but not limited to:
  • Spearheading grant work and submission related to the practice of pharmacy.
  • Lecturing and facilitating learning opportunities for pharmacy students, PGY1 and PGY2 pharmacy residents, and practitioners.
  • Performing duties on local, system. And national committees which contribute to the development and professional advancement of pharmacy practice.
  • Serving as a leader or facilitator on a regional or national professional organization.

Minimum Qualifications:

Education: Doctor of Pharmacy (PharmD) or Bachelor degree in Pharmacy .

Experience: Five (5) years as a clinical Pharmacist.

Licensed Pharmacist in the state of VA.

Certification: Basic Life Support (BLS), Advanced Cardiovascular Life Support (ACLS) for Critical Care and Emergency Medicine, and Pediatric Advanced Life Support (PALS) within 90 days of hire.

Preferred Qualifications:

• PGY2 in ambulatory care, cardiology, or BCACP eligible
• Experience in warfarin management inpatient or outpatient, including LVAD patients and periprocedural bridging.
