Job summary

Hopscotch Primary Care in North Carolina is seeking an Advanced Practice Provider to join our growing rural health team. You’ll deliver comprehensive primary care through in-person and telehealth visits, manage acute and chronic conditions, and collaborate with physicians, nurses, and care managers to improve patient outcomes.

You'll bring 2+ years in a primary care setting, active NP/PA licensure, and experience with HCC documentation, ICD-10 coding, and HRAs.

Qualifications

• Active NP or PA license in the state.
• 2+ years in a primary care setting.
• Experience with HCC, ICD-10, and HRAs.
• Ability to work with telehealth and EHR systems.

Responsibilities

• Provide comprehensive primary care in person and via telehealth.
• Manage routine, acute, and chronic conditions.
• Perform annual wellness visits and health risk assessments.
• Collaborate with physicians and care team to coordinate care.
• Apply evidence-based guidelines to deliver high-quality care.
