Job summary

Hopscotch Primary Care is seeking an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to deliver comprehensive, interdisciplinary primary care. You will conduct office and telehealth visits, collaborate with a diverse care team, and focus on patient outcomes and quality metrics.

Ideal candidates have 3+ years in primary care, active US licensure, and experience with HCC, ICD-10 coding, and HRA documentation.

Qualifications

• Minimum of 3 years’ experience in a primary care setting.
• Active, non-probationary state medical license (US).
• Experience with HCC documentation, ICD-10 coding and health risk assessments.

Responsibilities

• Deliver comprehensive primary care including office and telehealth visits.
• Collaborate with physicians, community health workers, case managers, pharmacists, and others.
• Complete wellness visits and risk assessments.
• Follow evidence-based guidelines and innovate care models.
• Meet quality metrics, patient experience, and utilization targets.
