Job summary

Rocky Mountain Health Care Services’ Rocky Mountain PACE Day Center seeks an Activity Coordinator to plan and facilitate person-centered recreational, therapeutic, and social activities for older adults in Colorado Springs.

You will collaborate with an Interdisciplinary Team to support participants’ independence, safety, and quality of life, while ensuring activities meet individual goals and adapt to diverse needs.

Qualifications

• Minimum of 1 year of experience working with older adults, preferably in PACE, adult day programs, long-term care, or memory care.
• Experience planning and facilitating activities for individuals and groups with varying physical, cognitive, and emotional needs.
• National certification in activity programming preferred; certification must be initiated within 120 days and completed within 12 months.
• Current CPR and First Aid certification or ability to obtain upon hire; background checks required.

Responsibilities

• Plan, schedule, and facilitate daily group and individual activities, including physical exercise, cognitive stimulation, arts and crafts, music, socialization, reminiscence, special events, and community-based programming.
• Design and adapt activity programming to accommodate participants with varying physical, cognitive, emotional, and behavioral health needs.
• Promote participant choice, engagement, and independence while fostering a welcoming, respectful, and inclusive Day Center environment.
• Coordinate theme days, special events, and approved community partnerships.
• Assist with participant escorting and community outings as assigned, ensuring safety and adherence to organizational and regulatory requirements.
