Job summary

Rocky Mountain Health Care is seeking an Activity Coordinator in Colorado Springs to plan and facilitate daily activities that promote physical, cognitive, emotional, and psychosocial well-being. You will collaborate with an Interdisciplinary Team to align activity programming with individualized goals and safety standards.

The role emphasizes person-centered care, adaptive planning for diverse needs, and active participation in client assessments and care planning within a PACE Day Center

Qualifications

• High school diploma or equivalent; associate degree preferred.
• Minimum of one year experience with older adults, preferably in PACE or memory care.
• Experience planning activities for individuals and groups with diverse needs.
• National certification in activity programming preferred; must be initiated within 120 days and completed within 12 months.
• Current CPR and First Aid certification or ability to obtain upon hire.
• Valid driver’s license and ability to drive for outings or transportation.
• Ability to pass required background checks and health screenings.

Responsibilities

• Plan, schedule, and facilitate daily group and individual activities (exercise, cognition, arts, music, socialization, reminiscence).
• Design and adapt activities to varied physical, cognitive, and emotional needs.
• Promote participant choice, engagement, independence in a welcoming Day Center environment.
• Coordinate theme days, events, and community partnerships.
• Assist with participant escorting and community outings, ensuring safety and regulatory adherence.
• Observe participant engagement, mood, behavior, and safety during activities.
