Job summary

TCN Behavioral Health Services Inc is seeking an ACT Case Manager to deliver intensive case management for adults with serious mental illness in Xenia, OH. You will work under clinical supervision to ensure timely notes, service planning, and linkage to supportive services within authorized timeframes.

The role requires a bachelor's in a human/social service field or an associate degree with equivalent experience, plus 3 years of SMD work, and certifications in First Aid, CPR, and de-escalation.

Qualifications

• Bachelor's degree in a human/social service field or an associate degree with equivalent field experience.
• Minimum 3 years working with the SMD population.
• First Aid, CPR, and De-escalation certifications.
• Valid Ohio driver's license and proof of automobile liability insurance.

Responsibilities

• Provide intensive case management under clinical supervision and meet productivity/quality standards.
• Accumulate required billable hours and maintain documentation within 24 hours of sessions.
• Develop and maintain individualized service plans and ensure continuity of care.
• Coordinate with Mental Health and AOD clinicians for multidisciplinary services.
• Maintain accurate clinical charts and participate in supervision and trainings.
