Job summary

TCN Behavioral Health Services in Xenia, OH is seeking a full-time ACT Case Manager. Provide individual case management under clinical supervision, ensuring progress notes are completed within 24 hours and that services align with agency standards.

You will collaborate with clinicians and other social service agencies to coordinate services, maintain up-to-date documentation, and travel within Greene County as required. A valid Ohio driver’s license and appropriate insurance are required.

Qualifications

• Education and experience requirements for SMD population.
• Certification: First Aid, CPR, De-escalation.
• Valid Ohio driver’s license and proof of automobile liability insurance.

Responsibilities

• Provide individual case management under clinical supervision.
• Meet or exceed agency standards and productivity requirements.
• Accumulate billable hours of clinically allocated time as per agency standards.
• Maintain accurate clinical charts and progress notes within 24 hours of sessions.
• Coordinate with Mental Health and AOD clinicians for multi-disciplinary services.
• Liaise between hospital, justice system and treatment services in the community.
