Job summary

Fallon Health is seeking an Accountable Care Organization Behavioral Health Case Manager II (BHCM II) to join an interdisciplinary team focused on transition of care, care coordination, and improved access to quality care for Fallon Health ACO members.

The BHCM II will work in partner provider offices, building telephonic and face-to-face relationships with members and caregivers to align with individualized care plans.

Qualifications

• Master’s degree in social work or related field.
• LCSW or MA/MS in Mental Health Counseling under clinical supervision.
• Certification in Case Management is a plus.

Responsibilities

• Conduct behavioral health assessments and develop comprehensive care plans addressing behavioral, medical, and social needs.
• Strong care coordination and member advocacy within interdisciplinary teams.
• Collaborate with providers and organizations to ensure timely mental health and substance use supports.
