Fallon Health, a Worcester-based nonprofit health care organization, seeks an ACO Behavioral Health Case Manager I to join an interdisciplinary team. You will perform assessments, develop individualized care plans, and coordinate with providers to improve access and quality of behavioral health services for Fallon Health ACO members.

The role requires a Master’s degree in a related field, licensing (LCSW) under supervision, and at least four years of behavioral health experience.

Qualifications

• Master's degree from an accredited program in social work, mental health counseling, or psychology.
• Experience with behavioral health case management and care coordination.
• LICENSING: LCSW or equivalent, under supervision.
• CORI check required and HIPAA compliance.

Responsabilités

• Conduct in-home and telephonic behavioral health assessments and develop care plans with the care team.
• Coordinate with providers, nurses, social workers, and community resources to ensure timely care.
• Maintain documentation in TruCare EMR and other systems; participate in clinical rounds.
• Support quality initiatives and ensure compliance with partner requirements.
