United Community Action Partnership, Inc. in Marshall, MN is seeking a Self-Sufficiency Case Manager for full-time, 40 hours per week, 12 months per year.

The role focuses on empowering individuals and families to achieve personal goals, assess needs, and develop action plans for housing stability and independence. You will document contacts, maintain confidentiality, collaborate with providers, and ensure timely reports while promoting self-sufficiency and community engagement.

Qualifikationen

• Two to four years of education and/or experience in a related field.
• A four-year post-secondary degree in Human Services in preferred.

Aufgaben

• Empower and support participants to set and achieve personal goals.
• Conduct assessments, develop individualized plans, and assist with housing stability.
• Keep detailed records, document contacts, and maintain confidentiality.
• Promote self-sufficiency and independence from public assistance.
• Build relationships with service providers, attend meetings, and advocate for participants.
• Stay organized by documenting, tracking referrals, and completing reports on time.
