A Self-Directed Services Support Broker, under general supervision and review, helps individuals in self-directed service programs design, manage, and control their own supports. Working on behalf of the individual, they focus on promoting choice, independence, and self-advocacy. They assist with person-centered planning, guide individuals in managing their service budgets, and support them in hiring and supervising staff. They assist in developing person-centered plans by helping individuals define their goals and translate them into actionable support, while also providing guidance on managing the individual budget associated with Medicaid-funded services. This includes helping allocate funds, monitor expenditures, and make adjustments as needed. While they collaborate with case managers, their role is distinctly separate and is centered on coaching, advocacy, and helping individuals navigate systems and make informed decisions, ensuring services are flexible, personalized, and aligned with the individual’s goals. A Self-Directed Services Support Broker completes, coordinates, and monitors agency documentation of services provided and performs related work activities as assigned.

An employee in this class generally works remotely or in the offices of HealthWest, although travel throughout the County by use of personal resources is required to provide face-to-face service to individuals in their home or community setting, facilitate or attend training, and/or for other required job duties.

Appointees must satisfactorily complete a probationary period before the appointment will be considered permanent.

The purpose of this recruitment is to establish an eligible list from which to fill present and future vacancies in this classification in the County system. For each opening, the Department Head has her/his choice of the candidates who meet the required minimum entrance qualifications. This list remains in effect for three months unless exhausted or extended.
