Job summary

Carbondale Rehabilitation and Healthcare Center, a skilled nursing facility in Pennsylvania, is hiring a Registered Nurse (RN) for second shift (3PM-11PM), Monday through Friday. You will deliver compassionate, high-quality care, administer medications, and lead a team of LPNs and CNAs to support residents’ well-being.

Qualifications

• Active and unrestricted RN license in Pennsylvania (PA).
• Previous experience in skilled nursing, long-term care, or similar setting preferred.
• Strong clinical assessment skills, attention to detail, and excellent communication abilities.
• Passion for geriatric care and a genuine desire to improve residents’ lives.
• Current CPR/BLS certification

Responsibilities

• Assess, plan, implement, and evaluate individualized resident care plans.
• Administer medications and treatments accurately and safely.
• Closely monitor residents’ conditions and promptly communicate significant changes to physicians and the care team.
• Provide leadership and support to LPNs and CNAs, fostering a positive and effective team environment.
• Maintain thorough, timely, and accurate documentation in compliance with all regulations.
• Partner with interdisciplinary teams to deliver coordinated, holistic care.
• Respond swiftly and effectively to emergencies with calm, competent interventions.
• Educate residents and families on health conditions, treatment plans, and wellness strategies
