Job summary

City of Seattle's Human Services Department is hiring an Outreach Counselor to support the Unified Care Team’s daily operations in Seattle. You will deliver direct services to individuals experiencing homelessness, assess needs, and connect clients with housing, behavioral health, medical care, and employment resources.

Qualifications

• Three years of social services experience involving interviewing, counseling, or crisis intervention.
• Bachelor’s degree in Social Services, Psychology or related field or equivalent combination of education and experience.
• Current Washington State Driver’s License.

Responsibilities

• Conduct individualized needs assessments for people in encampments within your district.
• Develop service plans and refer individuals to appropriate resources.
• Provide motivational interviewing, de-escalation, and crisis intervention.
• Collaborate with contracted partners to ensure appropriate levels of care.
• Navigate homelessness services and assist with transportation arrangements.
• Maintain concise case notes and enter data in multiple platforms.
• Engage with Neighborhood Outreach Team and report progress to the Regional Coordinator.
