Responsibilities

• Coordinate with nurses/providers and hospital teams.
• Screen/assess social needs for primary care patients.
• Coordinate social services resources (housing, clothing, food, mental health, etc.).
• Collaborate with other social workers to identify resources.
• Conduct case management activities and discharge planning.
• Assist in obtaining patient records from hospitals and specialists.
• Assist in securing medical equipment through community partners.
• Follow up on care plans and track referrals.
• Identify patients lost to follow-up and re-engage them in care.
• Assist with patient assistance program applications and drug company liaison.
• Provide patient education and connect to education resources.
