Job summary

STI is seeking a Rise Navigator to support screening, assessment, crisis intervention, and linkage to recovery services in Hancock and Hawkins counties, TN. The role involves collaboration with community partners, outreach, and documentation in secure records systems.

The contract position requires at least 18 years of age, a high school diploma or GED (Bachelor’s degree preferred), and about one year of professional experience. Travel and background checks are required.

Qualifications

• Be at least 18 years old.
• Hold a high school diploma or GED or equivalent.
• Bachelor’s degree preferred or equivalent experience in substance use/social work.

Responsibilities

• Facilitate SUD screenings and assessments using validated tools.
• Provide crisis intervention and counseling in health department settings.
• Educate to prevent overdose deaths and disease transmission.
• Distribute naloxone and engage in non-traditional outreach.
• Build partnerships with community resources and SSPs.
• Link clients to mental health, addiction treatment, and recovery services.
• Document progress notes and referrals; maintain compliant records.
• Attend team meetings and trainings; support continuous improvement.
