Polk County Schools is seeking a Mental Health Facilitator (11-month) to coordinate referrals, oversee student case management, and provide direct mental health services within schools in the district.

The role supports Tier I-III services, collaborates with administrators, teachers, and district teams, and implements mandated safety and mental health plans. Requires relevant degree and licensure preference.

Qualifications

• Bachelor’s degree in mental health or related field required; master’s preferred.
• Licensure as a mental health provider preferred (e.g., School Psychologist, Professional School Counselor, LMHC, LCSW).
• Experience gathering/analyzing data; crisis intervention and collaborative teaming required.

Responsabilités

• Coordinate referral process for students needing mental health services beyond the school level.
• Provide individual student case management oversight for Tier III needs.
• Deliver mental health and behavior supports, coaching, and training to schools in the region.
