Job summary

Polk County Schools seeks a Mental Health Facilitator (11-mo) to support the SPARK Resiliency initiative in middle schools. Based at Dwight Smith Center in Lakeland, this itinerant role coordinates student referrals, access, and delivery of mental health services as a liaison between the school and district.

Responsibilities include collaborating with school staff, coordinating Tier III referrals, providing direct counseling, and assisting with the district's safety plans, while PREPaRE

Qualifications

• Bachelor's degree in a mental health field required; master's preferred.
• Certified mental health provider; licensure preferred (e.g., school psychologist, counselor, MSW).
• PREPaRE certification must be obtained within one year of placement.
• Experience in data gathering and crisis intervention, collaborative teamwork.

Responsibilities

• Coordinate referrals and oversee case management for students needing Tier III services.
• Provide direct mental health counseling and supports to students and staff.
• Maintain documentation including insurance/Medicaid claims for services.
• Collaborate with administrators, teachers, and school-based mental health teams.
• Support implementation of the district's safety and intervention plans.
