Job summary

Polk County Public Schools seeks a Mental Health Facilitator (11-month) to coordinate student referrals, access, and delivery of mental health services between the school and district. The role acts as a liaison ensuring timely support and collaboration among administrators, teachers, and school-based teams.

The position requires a bachelor’s degree in a mental health field (master’s preferred) and certification readiness.

Qualifications

  • Degree in a mental health-related field required; master's preferred.
  • Experience in gathering and analyzing data is preferred.
  • Crisis intervention and collaborative teaming experience is required.

Responsibilities

  • Maintain collaborative relationships with administrators, teachers, and school-based mental health teams.
  • Coordinate the referral process for students needing mental health services beyond the school level.
  • Provide case management oversight for students in need of Tier III services.
  • Provide mental health and behavior supports, coaching, and training to schools in the region.
  • Participate in intervention plan monitoring per threat and risk assessment processes.
  • Provide direct service mental health counseling.
  • Assist in implementing the school safety act plan and related requirements.
  • Document all services provided to students, including insurance or Medicaid claims.
