Job summary

The Coordinator will maintain records, coordinate with clinicians and school staff, and support reporting for grants, while traveling to schools and clinics using a personal vehicle.

Qualifications

• Requires completion of patient services training.
• Athena training or equivalent health information systems training preferred.
• Ability to work independently in community settings and coordinate with schools and clinics.
• Proven ability to develop partnerships with school staff and community organizations.

Responsibilities

• Serve as liaison between school administrators and the Mobile Van to coordinate schedules and service delivery.
• Ensure proper consent forms and authorizations are obtained prior to healthcare services.
• Maintain medical records, collect data, greet students and families.
• Verify provider certifications with Chicago Public Schools and background checks.
• Coordinate with School and Medical staff and attend meetings as needed.
• Assist with registration, consent forms, and front desk processes.
• Ensure Mobile Van is stocked with supplies and forms.
• Train in EMR entry and proper patient coding before visits.
• Protect client confidentiality per HIPAA/PHI rules.
• Prepare monthly reports for grantors and maintain program metrics.
• Submit reports to the Director of Clinical Services and stakeholders.
• Coordinate school visits to clinics and participate in meetings.
