Job summary

Sage Connections, LLC in Elgin, IL is seeking a dedicated Registered Nurse (RN) for 1:1 student support in a school-based setting for the 2026-2027 school year. This onsite role offers Monday through Friday schedules and meaningful collaboration with educators and families.

The ideal candidate will hold an active RN license, Pediatric/school nursing experience, CPR certification, and strong communication and clinical decision-making skills.

Qualifications

• Active RN license in good standing (state-specific).
• Pediatric or school nursing experience preferred.
• Knowledge of medication administration, feeding tube care, and blood glucose monitoring.

Responsibilities

• Provide direct 1:1 nursing support during the school day.
• Assist with activities of daily living (ADLs) as needed.
• Monitor and manage student health needs, medications, feeding support, and routine health checks.
• Support students with mobility and transfers as required.
• Collaborate with school staff and families to ensure care plans are followed.
• Maintain accurate documentation of nursing care and student health status.
• Respond to medical emergencies according to established protocols.
