ProCare Therapy seeks a licensed Physical Therapist to provide school-based PT services onsite near Espanola, NM. You will manage a small caseload across multiple campuses and travel between sites within the district, delivering evaluations and interventions aligned with IEPs.

Qualifications

• Licensed PT with school-based experience preferred.
• Strong understanding of school-based PT services, assessment, and documentation.
• Ability to manage a small caseload.
• Willing to travel between school sites within the district.
• Available to start as soon as possible.
• Open to both full-time and part-time availability.
• Excellent communication, organization, and time management skills.

Responsibilities

• Deliver physical therapy interventions and evaluations onsite to students.
• Collaborate with teachers, administrators, and families to create and implement individualized education plans (IEPs).
• Travel between assigned school locations within the district to provide consistent therapy coverage.
• Maintain thorough documentation and communicate progress effectively.
• Participate in IEP meetings, student evaluations, and multidisciplinary team discussions.
