Job summary

Constellation Kids is seeking a School-Based Physical Therapist to provide evaluations, interventions, and lead individual and small group sessions in collaboration with school teams. The PT will document outcomes, participate in IEP meetings, and communicate effectively with educators, parents, and administrators.

The role supports transitions across programs, may supervise fieldwork students, and requires CT licensure.

Qualifications

• Master's or Doctorate in Physical Therapy.
• Connecticut State License required.
• Field work in pediatrics preferred.

Responsibilities

• Gather information needed to evaluate a child by reviewing records and communicating with the school team.
• Administer appropriate assessment instruments and procedures, considering age and developmental level.
• Interpret assessment results to develop or modify PT intervention plans.
• Plan and implement intervention strategies using direct therapy, monitoring, and consultation.
• Participate in the IEP process and engage in consensual decision making with the school team.
• Provide accurate documentation of assessment and intervention results.
• Facilitate transitions among agencies and programs as service provision changes.
• May supervise Level I/ Level II field work students and mentor PTA.
