Job summary

Constellation Kids is seeking a School-Based Physical Therapist in Stamford, CT. You will conduct evaluations and deliver PT interventions within school settings, leading individual and small group sessions, and working closely with school teams and other therapists.

You will attend IEP meetings, document assessment results, and assist with transitions between programs. Newly licensed PT grads are encouraged to apply, with opportunities for mentorship and career growth.

Qualifications

• Graduate of an accredited college or university with a Master’s or Doctorate degree in Physical Therapy.
• Licensed as a Connecticut Physical Therapist.
• Field work in pediatric setting preferred.

Responsibilities

• Perform evaluations and interventions for school-age children.
• Lead individual and small group therapy sessions.
• Collaborate with school teams and other therapists; participate in IEP meetings.
• Provide documentation of assessments and interventions.
• Facilitate transitions between agencies and programs as needed.
• May supervise Level I/II fieldwork students and mentor newer PTs.
