Job summary

I3 INFOTEK INC is seeking a licensed Occupational Therapist (OT) to provide school-based occupational therapy services in Phoenix, AZ. The role focuses on evaluating students, creating individualized treatment plans, and supporting their educational participation.

The OT will collaborate with educators, families, and a multidisciplinary team, maintain progress documentation, and participate in IEP processes. Arizona licensure and pediatric experience are required.

Qualifications

• Graduation from an accredited OT program is required.
• Arizona OT license is required.
• NBCOT certification is preferred.
• Minimum 2 years pediatric OT experience.

Responsibilities

• Perform occupational therapy evaluations and assessments.
• Develop and implement individualized therapy plans.
• Write and monitor IEP goals.
• Provide direct therapy services to students with disabilities.
• Collaborate with teachers, parents, and other educational professionals.
• Maintain accurate therapy documentation and progress reports.
• Train school staff on therapy techniques and student accommodations.
• Participate in Medicaid in Public Schools (MIPS) documentation and billing.
• Serve as a resource for students with sensory impairments and complex needs.
