School-Based Nurse Practitioner — Patient Care Lead

Job summary

SWLA Center for Health Services, Inc. seeks a Nurse Practitioner to join the Medical Department at Lafayette Renaissance Charter School. You will provide ambulatory care, perform exams, diagnose and treat, and collaborate with physicians under a collaborative agreement.

The position follows the school year (August 1–May 31) with hours 7:30am–4pm, Monday–Friday. A valid Louisiana NP license and DEA, plus BLS/ACLS, are required.

Qualifications

• Graduation from an accredited nursing or NP program.
• APRN certification and LSBN CEU requirements.
• Current Louisiana nursing license and APN certification.
• Experience in writing policies/procedures.
• Proficient with MS Word and Excel.
• Ability to relate effectively with Health Center staff.
• Strong critical thinking and problem-solving abilities.
• Sufficient experience to perform duties of NP role.
• Knowledge of Health Center practice management/EMR.

Responsibilities

• Obtain complete medical history from patient/family/records.
• Prescribe medications per SWLA protocols and explain treatment.
• Assess, diagnose, treat, monitor health status.
• Perform complete physical examinations.
• Order routine labs and imaging per standing orders.
• Diagnose acute/chronic illness per protocols.
• Develop and monitor patient management plans with supervising physician.
• Serve as Team Leader of SWLA Redesign Team.
• Refer patients to physicians/specialists as needed.
• Provide emergency care and resuscitation when required.
• Consult with physicians and health care team as necessary.
• Adhere to applicable laws and standards of conduct.
• Maintain patient records with required signatures.
• Attend medical staff meetings and quality programs.
• Participate in performance improvement activities.
• Meet goals established for the employee.
• Consult daily with CHO/physicians for HRSA/NCQA requirements.
• Ensure redesign teams are properly staffed.
• Monitor patient flow to meet redesign benchmarks (<45 min).
• Take on additional duties as needed.
• Suggest policy changes to improve coverage.
