Job summary

CONCERN seeks a Partial Hospitalization Mental Health Worker to support students in the Elkland High School program in Mansfield. Working under the Mental Health Professional, you will provide individual and group sessions, track progress, and coordinate care with school staff within school, home, and community settings.

Requirements include a Bachelor's degree in a related field or equivalent experience; valid driver's license and clearances.

Qualifications

Bachelor's degree or higher in Sociology, Social Work, Psychology or related field.
Alternative: associate degree with 1 year experience in behavioral/mental health.
High School diploma with 2 years behavioral health experience.

Responsibilities

Observe clients, implement treatment interventions, and provide case management as needed.
Lead group and family sessions in school, home, and community under supervision of MHP.
Coordinate care with other service systems and assist with staff liaison activities.
Record data, track progress on objectives, and complete required charting and audits.
Attend MHP staff meetings and maintain confidentiality and licensing standards.
Attend required trainings per program requirements.
