Qualifications

• High School diploma required; Associate degree preferred.
• Community Health Worker Certification required or to be obtained within designated timeframe.
• Experience with community resources, healthcare systems, or schools is preferred.

Responsibilities

• Coordinate school-based tele-behavioral health services and schedules.
• Assist students and families with enrollment in health programs and services.
• Facilitate communication between families, schools, providers, and community resources.
• Document interactions accurately in the EMR and maintain HIPAA confidentiality.
• Support outreach, enrollment, and patient navigation activities.
