Job summary

The Child Center of NY is seeking a Family Coordinator for the School-based Early Support Program to provide comprehensive support and intervention services to children and families within the school setting.

You will assess needs, develop individualized plans, coordinate services with school staff, families, and community partners, and advocate to promote well-being and academic success.

Qualifications

• Bachelor’s Degree in Social Work, Psychology or Social Services.
• At least 2 years’ experience in related field.
• Good interpersonal skills and ability to work with diverse groups.
• Proficiency in Microsoft Office.
• Ability to plan and carry out assignments independently.
• Ability to prioritize, adhere to timelines and multi-task.

Responsibilities

• Assess and plan for children and families, identifying strengths, needs and goals.
• Collaborate with school personnel and stakeholders to develop comprehensive support plans.
• Coordinate access to educational, mental health, medical and social services.
• Advocate for children and families within school and community settings.
• Document case records, progress notes and required reports with confidentiality.
• Engage in training and professional development related to child development and cultural competency.
• Perform other duties as assigned.
