Job summary

Sheppard Pratt is seeking a School-Based Case Manager to provide culturally and linguistically appropriate case management at a high school, in collaboration with MCPS and DHHS, to promote self-sufficiency and linkage to mental health resources for un- and under-insured youth and families.

The role requires a Bachelor's degree in Social Work or related field, experience with school-based models, and preference for English/Spanish bilingualism.

Qualifications

• Bachelor’s degree in Social Work (B.S.W.), Psychology, or related field. Internship experience acceptable.
• Experience with collaborative school-based models is preferred.
• Knowledge of immigrant populations and acculturation issues is preferred.

Responsibilities

• Provide case management services to students/parents/families with economic and/or social needs (housing, healthcare, employment, food, clothing, education).
• Document service plans in the County’s electronic records system and update every six months.
• Maintain regular contact with clients (at least twice a month) and ensure data entry within one business day of service delivery.
• Maintain a caseload in compliance with contract terms and agency expectations.
• Organize and facilitate meetings and events to develop partnerships between families and schools.
• Develop referral systems with service providers to promote continuum of care.
• May conduct home visit meetings as necessary and make referrals as needed.
• Collaborate with therapist, school staff, and community partners to provide comprehensive care while maintaining confidentiality.
